More than 22 million children under five are now obese or overweight. Globally, an estimated 10% of school-aged children, between 5 and 17 years old, are overweight or obese, and in some countries this figure is as high as 35%. The situation is getting worse. Although recognised clinically for some time as an important condition that increases risk of ill-health in affected individuals, it is only recently, that obesity has been recognised as a population-wide problem that requires preventive action. Obesity is a major contributor to diseases and disability, the associated health costs are enormous, obesity has already reached epidemic proportions in many countries, and incidence is continuing to increase in children and adults. Disturbingly the epidemic is not confined to developed countries, with many developing countries and those in transition affected. While recognised as a major population health problem, our understanding of the causes of the epidemic is poor, there has been relatively little population-based research that has focused on the prevention of unhealthy weight gain, and as a consequence knowledge regarding how and where best to intervene is limited. This book draws together the existing literature and expertised with a view to helping set the agenda for public health action. The book is divided into three sections. Part 1 provides an overview of the context of the problem. It examines the epidemiology of obesity, the role of behavioural factors, socio-cultural factors and environmental factors in the obesity epidemic. Part 2 reviews interventions across a range of key settings and in different population groups - drawing on existing research that has aimed to increase physical activity, promote healthy eating and prevent obesity at a population level. Given how little research there is that has specifically examined the effectiveness of interventions aimed at preventing obesity per se, Part 3 explores potential opportunities to prevent obesity.

Blood type solution found, say scientists

Scientists say they have discovered a simple way of converting blood from one group to another, which could help reduce blood-supply shortages around the world.

O-type blood can be transfused safely into humans of any blood type without medical risk.

The problem is that it is not always readily available for those who desperately need blood.

An international team of scientists now believes it has found a way to convert A, B and AB blood types into O-type, making it safe to use in transfusions.

The process promises not only to increase the stock of blood available for transfusions but could also prevent the accidental transfusion of the wrong blood group into patients.

Blood types are based on the type of a molecule, called agglutinogen, found on the surface of red blood cells. Types A, B, or AB can clump together if mixed, but type O does not react with other types.

More than two decades ago, researchers began testing the concept of removing the surface proteins on red blood cells to create universal blood cells, according to the report in the April 1 online issue of Nature Biotechnology.

In 2000, findings from a trial showed that type B red blood cells could be converted into type O cells, which then survive normally when given to type A and O individuals. However, the usefulness of this approach was limited by the large amount of enzymes required for the conversion.

In the present study, Dr Henrik Clausen and colleagues from the University of Copenhagen in Denmark describe two previously unknown enzymes, small amounts of which are capable of efficiently removing the A and B agglutinogen.

"The enzymatic conversion processes we describe hold promise for achieving the goal of producing universal red blood cells, which would improve the blood supply while enhancing the safety of clinical transfusions," Dr Clausen and his team concluded.
